Event timeline & details
You should complete this form at least two weeks before your event. This form will be used by Harold and his team to plan out coverage and details for your special day.
Print release
You should download this form if you are planing on printing your own images or album. Retailers like CVS, Walgreen, Costco etc may require a print release. Your print release number is your event date (YYYYMMDD).
Insurance form
You should download this form if required by your venue or shooting locations to have insurance. This form is only valid for events shot by Harold Duarte and his team.
